Hospital Guide in Fukuoka for Foreign Residents is available on Rainbow Plaza’s website. You can search according to type of treatment, language, or location, so please make use of this service. Some hospitals require an appointment, so please ask them beforehand. As medical expenses can be quite expensive, you should consider joining a foreign travel insurance before your visit to Japan, or if you are registered for alien registration and living in Japan for over a year, enroll for Japanese National Health Insurance as soon as possible.

Receiving Treatment at a Clinic or a Hospital
At the reception desk, state your name, reason for the visit, and whether it is your first visit, and present your insurance card. From your second visit, present your patient card (shinsatsuken). However, since it is required to show your health insurance card every first visit of the month, you should have the card with you. Afterwards, wait until your name is called. After receiving treatment, pay your bill at the pay desk. If you are prescribed medicine, present your prescription (shohosen) at the pharmacy (usually by the hospital), and receive your medicine upon paying. Some hospitals have on-site pharmacies. In that case, your prescription is forwarded directly to the pharmacy from the hospital, so pay the combined fee and receive your medicine
* Caution:
On your first visit, first visit fee will be charged in addition to your medical bill. Even if you are receiving treatment for the same illness, if the time between your visits exceeds one month, it is considered as a first visit. If you are visiting a general hospital with over 200 hospital beds and various medical services, an additional fee aside from the first visit fee will be charged. (This fee is not necessary if you were referred to this hospital by your local doctor with a referral letter, “Shoukaijou”, or if you were brought to this hospital by an ambulance.) Also, there are programs offered such as the benefit for high-cost medical care in your health insurance or the Medical Cost Deduction from your income tax, which can be used when paying medical fees above a certain amount. For these purposes, please retain your receipt of medical fees.

Ambulance
In case of an emergency, call 119 for assistance. When using a payphone, be aware that some phones are required to push the emergency button before dialing 119. When the operator answers the phone, state the following.
1)Say, “Kyukyusha wo onegai-shimasu.” (please send an ambulance.)
2)State your name and location (address or detailed description of whereabouts).
Speak clearly and slowly. To avoid misunderstanding, give the spelling of words if necessary.
Give your age, gender, and a description of your location using landmarks (for example, “I am at the front entrance of Fukuoka Tower.”) Describe your condition (e.g. I have a broken leg). If possible, speak in Japanese
Emergency Clinics for Holidays and After-hours
In Fukuoka City, the following clinics are open for emergency treatment on Sundays, holidays, and after-hours. However, types of treatment and opening hours are restricted, so if you wish to receive a type of treatment not offered, contact the Fukuoka Prefecture Emergency Medical Information Center for the nearest medical institution that can help you. Remember to take your insurance card or Infant Medical Care Certificate with you.

<Major Types of Public Health Insurance and its Insurers >

Registration and Premiums
For company employees, the employer makes arrangements for health insurance, and premiums are paid half by the employer and the other half by the employee. For details, please ask your employer. For National Health Insurance, you will have to make arrangements and pay premiums on your own. For details, please consult the health insurance and pension department of your ward office. After enrolling in the National Health Insurance plan, you will be given a Health Insurance Card. If you show this card when visiting a hospital, you will only have to pay 30% of the total fee. However, there are certain treatments not covered by this insurance. It is recommended that you check with the hospital beforehand. The amount of the premium is set according to your income.
Pregnancy/Birth
When you become pregnant, submit a notification of pregnancy to a Health and Welfare Center (public health center) or an obstetrician, and you will receive a Maternal and Child Health Handbook. Keep this handbook, as it is used to record important data such as the course of your pregnancy and your child's growth and development, and is used as a reference for your child’s physical examination when entering elementary school. Health and Welfare centers also provide various health services for expectant mothers and infants.

If you are registered as a foreigner in Fukuoka City, they will distribute a foreign language version for free upon request.

Foreign language versions of the Maternal and Child Health Handbook are issued by the Mother’s and Children’s Health and Welfare Association (Tel: 03-4334-1188), and are available in English, Chinese, Korean, Portuguese, Spanish, Indonesian, Thai, and Tagalog (750 yen, shipping and tax not included). The Mother’s and Children’s Health and Welfare Association also issues foreign language versions of the Maternal and Child Health, Medical Care and Child Rearing in Japan (500 yen, shipping and tax not included).
Notification of Birth
If one of the parents has Japanese nationality, the child can obtain Japanese nationality as well. After your baby is born, submit a notification of birth to the citizens department of your ward office within 14 days of birth. You will need to bring the Maternal and Child Health Handbook, doctor’s certificate of birth, and personal seal.
If both of the parents are foreign nationals and the baby is staying in Japan for over 60 days, apply for a status of residence at the immigration bureau within 30 days; if the baby is staying in Japan for over 90 days, apply for alien registration at the citizens’ department of your ward office within 60 days.
Remember to also register the birth with the embassy of your home country
Medical Care Certificate for Infants
After your baby is born, immediately enroll in public health insurance (i.e. Health Insurance, National Health Insurance etc.) and Medical Care Certificate for Infants. Medical Care Certificate for Infants is needed to receive financial assistance for the baby’s medical expenses. For details, please consult your employer or the health insurance and pension section of your ward office.
Coverage of Birth-Related Expenses
Normal births are not covered by insurance. However, you may receive financial assistance after giving birth upon application. For details, please contact your employer or the health insurance and pension section of your ward office.
Various financial assistances are available for your child until he or she enters elementary school. Contact the Child Rearing Support section of the ward office.

<Lump-Sum Allowance for Childbirth and Nursing> |

If you or your spouse is enrolled in Health Insurance, you will receive this allowance for each birth. This covers cases of miscarriages or stillbirths for pregnancies exceeding 12 weeks. You will receive 350,000 yen per child (to which 30,000 yen may be added). |

<Medical Fee Assistance Program for Infants> |

This assistance covers the non-deductible portion of health insurance for children until they enter elementary school (meals during hospitalization, or other expenses not paid by health insurance are not covered). Receive this benefit by presenting your insurance card and Medical Care Certificate for Infants at the reception desk of the medical institution. |
Child-Rearing Consultation
Advice on child-related issues such as physical development, discipline, breast-feeding, and diet, as well as the mother’s uncertainties and concerns are available. (In Japanese)
